Responsible Healthcare AI is not about better models; it is about safer systems. We articulate governance frameworks that describe the transition from stochastic probability to clinical determinism using the 6-pillar RATSe Framework.
IRHAI’s foundational position is that healthcare AI is unsafe unless probabilistic intelligence is governed by deterministic decision authority, accountability, and lifecycle control.
The Context: The Stability Gap
Healthcare AI rarely fails on benchmarks. It fails in deployment due to "probabilistic drift." The chart below illustrates the fundamental divergence between Standard AI (which optimizes for capability) and Governance-Aligned AI (which optimizes for stability).
Output Stability Over Repeated Clinical Queries
Measurement Definition: "Replication Rate" is defined as the temporal stability of clinical advice across identical inputs over time and across model updates. Ideally 100% for decision logic.

Decision Boundary
IRHAI explicitly rejects autonomous clinical decision-making by probabilistic AI systems.
AI may inform, recommend, or explain—but it must not independently initiate, finalize, or execute clinical actions without accountable human oversight.

Patient Primacy
The patient is the central and non-negotiable stakeholder.
Improvements in efficiency do not justify patient harm. Trade-offs must be explicit, clinically justified, and governed.
Risk Follows Clinician
Responsibility cannot be delegated to automation.
Risk owners must retain meaningful control and the ability to override AI outputs. Eroding agency introduces safety risk.
Stability Is Safety
In healthcare, instability causes harm faster than inaccuracy.
Predictable, reproducible, and traceable behavior is essential for systems operating at clinical scale. Deterministic decision pathways are therefore required wherever AI influences clinical actions, ensuring identical inputs produce identical outcomes, auditable logic, and enforceable accountability.

The IRHAI Governance Policy Spine (7 + 1)
IRHAI operationalizes its principles through a fixed set of seven core governance policies, with an eighth optional policy issued only when external regulatory or institutional pressure requires a standalone redress framework.
These policies do not function as compliance checklists or regulatory instruments. They define governance boundaries, not implementation instructions.
Scope & Limitations
The IRHAI policy set is intentionally limited in number, frozen in scope, and healthcare-specific by design.
Its purpose is to make responsibility enforceable at the point of clinical decision-making, not to replace regulators or certification regimes.

Optional Policy 8 (Redress): An optional eighth policy on human override, contestability, and redress is issued only when these requirements cannot be sufficiently embedded within the core accountability and lifecycle policies.
Governance-Aligned Architecture
To bridge the "Determinism Gap," the framework advocates for a Neuro-Symbolic approach. This separates Prediction (ML) from Decision (Symbolic Logic). LLMs serve as language interfaces, while clinical facts are processed through deterministic logic layers.
